When hospice documentation doesn't clearly show that a patient meets hospice eligibility criteria, claims to get paid for providing hospice care are denied at medical review. Learn to use assessment tools to provide measurable indicators that hospice patients are declining. Add these tools to your documentation kit: PPS, KPS, FAST, NYHA class IV and ECOG. Understanding how these standardized, validated tools work can enhance your hospice eligibility documentation and, if you use them along with patient-specific descriptions, can help build the case that your patient was appropriate for hospice services when you provided such are.
