Our mission is to promote the value and quality of end-of-life care for patients and their families through education and advocacy.
The majority of Americans (80%) say they would prefer to be cared for and die at home and yet only 38% of us receive hospice care. CHAPCA believes the best time to learn about hospice, and ask about hospice, is before hospice services are needed. To fully benefit from hospice services patients and families must be referred with a six-months prognosis to benefit from hospice services.
CHAPCA is a not-for-profit organization with a mission to improve access to quality end-of-life care and educates patients, families, the community and healthcare professionals about hospice and palliative care. It's our goal to support the mission of providing high quality hospice and palliative care services to patients and their caregivers and to provide leadership in advocating on behalf of hospice and palliative care in California.
