We, Brenda and Nancy, are two hospice nurses with over 40 years of experience at the bedside and are both certified hospice and palliative care nurses.  Our mission is to transform end of life care through education and support.  We have extensive experience teaching and mentoring students, nurses, volunteers, and the community at large.  This webinar course is based on our booklet "The Care Plan: A Path to Comfort for the Hospice Patient" which provides a new standard for in-home educational materials, and is organized by symptom management and personal care needs.  Our free videos on both our website, Thecareplan.net and YouTube are reaching thousands of people needing hospice information and clear hands on personal care instruction.

CHAPCA recently interviewed Brenda and Nancy as they gave a brief introduction of their publication entitled: The Care Plan: A Path to Comfort For The Hospice Patient". We recorded an introduction video to showcase the contents of the booklet and how hospice agencies can use it to educate caregivers, use as a tool for training nurses new to end-of-life care, and utilize the FREE caregiver videos to assist at every stage. This booklet provides a new standard for in-home educational materials and is organized by symptom management and personal care needs. It will help improve your CAHPS scores and meet the new Medicare requirements.
